Would you like to occasionally participate in exercise and wellness classes/activities/events with a group of your peers?

(Yes/No)

Question 3:

Despite being in otherwise good health, do you require assistance with transportation to appointments, scheduled check-ups, and weekly grocery shopping?

Question 4:

Do you require social interaction with a group of your peers?

(Yes/No)

Question 5:

Is your need for living independently more vital to you than your need for routine senior care?

(Yes/No)

If you’ve answered ‘Yes’ to more than three questions, it may be an indicator that you’re ready for assisted living.
